How To Order Supplies:
Log into QuickBase and go into 4.0 Practice Portal OR click button below:
Go to the AMD Orders button on the above navigation.
Click the green button New Order.
Fill in the required fields.
In the Order Items section, double click on the box under the Description column to select an item.
Indicate the quantity. This will populate ALL other fields.
Be sure to list ALL of the items you wish to order before clicking Save.
